Before Booking your Lessons,
Please Fill out the following Form. 

This form is simply for us to better get to know you and your swimmer!

Please fill it out once per swimmer and include any details needed to best accomodate you.

Swimmer Info Form:

We will review your form prior to your scheduled lesson(s) and reach out with any follow-up questions or additional information needed.

All swimmers and families will also be required to complete a waiver and obtain proper pool-use approval through the Pool Release Form before lessons begin.
